There are a number of reasons that may result in you needing to make changes to your NDIS plan:
1. My situation has changed, and my NDIS plan doesn’t suit my needs anymore.
2. My contact details have changed (for example, my phone number or address).
3. I am going to run out of funding before my NDIS plan expires.
In all three cases, you will need to complete a Change of Circumstances through the NDIS.
If you have a Support Coordinator, contact them for assistance to update your change of circumstances.
If you don’t have a Support Coordinator, you will need to complete the Change of details/situation form and submit it to enquiries@ndis.gov.au.
Within 21 days after lodging your Change of Details application, the NDIA will reassess your plan and contact you or your Support Coordinator to:
1. Confirm that they have made the requested changes
OR
2. Advise of the other options or next steps.
If you’re not happy with your NDIS plan, you can request an internal review of a decision.
If you have a Support Coordinator, contact them for assistance to request an internal review of your NDIS plan.
If you don’t have a Support Coordinator, you will need to complete the Request for a Review of a Decision form and submit it to enquiries@ndis.gov.au, along with supporting evidence.
If you’re still not happy with the outcomes of the interview review, you can request an External Review at the Administrative Appeals Tribunal (there is a financial cost).
