Our Support Coordinators are here to help you understand and use your NDIS plan effectively to live more independently in your community. We pride ourselves on our personalised support coordination approach. We work closely with you to not only understand your plan and your goals but to also get to know you and to know how you like things done.

Engaging with not only you as the participant but also taking into consideration your family needs/ formal supports and informal supports. Our experienced team pride themselves on their extensive knowledge of the NDIS policies and procedures. The team is never more than a phone call or email away and we also offer in house catch ups or meetings at a location convenient to you, should you want to meet in person.

 

Having had many years in the industry our Support Coordinators have experience in multiple diagnoses, disabilities and mental health conditions and have access to an array of allied health professionals. The team have also worked with and are proud of their knowledge of funding for various living arrangements including Specialised Disability Accommodation (SDA) and Supported Independent Living (SIL).

A Support Coordinator is funded by the NDIS to assist you to build the skills you need to understand and use your plan, however we take this further by ensuring that the supports you choose to engage with are right for you. Getting to know you and how you want to engage with providers is something our team takes seriously; it’s your choice and control and we strive to maintain that.

Their role includes things like assisting with personal care, household tasks, transportation, access social services, etc. In contrast, a support coordinator helps participants to understand and implement their NDIS plan. This may include selecting appropriate supports and services, goal setting, monitor plan budgets and support effectiveness.

Firstly, to receive the help of a support coordinator you need to request funding. You will also need to explain why you need this extra support. If your support coordinator funding is deemed ‘reasonable and necessary’ you will be granted funding. The next step is to source a support coordinator.
